All Insurer information has been made available to me and that I am aware that this policy is underwritten by Guardrisk Insurance Company Limited and administered by Viva Cover (PTY) Ltd, authorised Financial Services Providers. I agree to the cover limits, waiting periods, limitations and other terms of this policy as per the product terms and conditions. My policy documents will be emailed to me on the email address that I have provided within the next thirty (30) days. I further declare that all the information entered by me on my behalf is true and correct and should any further information be required; I will make this available to the administrator as necessary for my policy or any query related to the policy. Compli-Serve KZN is appointed by Viva Cover to ensure their compliance and I may contact Compli-Serve KZN for any compliance related queries.
Viva Cover (PTY) Ltd will advise me of the acceptance of this policy and if there are any terms and conditions that require additional disclosures for my policy.
I hereby authorise Viva Cover (PTY) Ltd to deduct premiums or any amounts as per the policy wording or terms and conditions of the parties. I acknowledge that failure/ rejection of said debits may result in my policy being suspended or cancelled. I agree that all payment instructions issued by the administrator will be treated by my nominated bank as if the instruction has been issued by me personally.
I am responsible for the payment of the premiums and the premium is payable monthly in advance, inclusive of Value Added Tax. If another person makes the premium payments on my behalf, that person will not have any right or entitlement to the benefits payable. A full month’s premium is due in respect of any insured device where cover commences or ceases during a calendar month.
If a premium is not paid on the specified payment date Viva Cover will notify me by SMS. If a premium is not successfully collected, Viva Cover may attempt to debit my account on an alternative date using the banks NAEDO system (non-authenticated early debit order) or a restrike before my next payment date and in the event of a successful debit my payment date may be amended and Viva Cover may keep debiting me on that same date in the future. If a premium is not paid on the due date, double the premium will be debited in the following month and if the payment date falls on a Saturday, Sunday or public holiday, Viva Cover will collect the premium on a more suitable business day. If the premium remains unpaid (after the double debit attempt) and I fail to remedy the failure within thirty (30) days (the Grace Period), the policy will lapse at midnight on the last day of the preceding period of insurance. All arrear premiums must be paid before cover shall resume in terms of this policy. In the event of a claim for benefits, Viva Cover shall be entitled to deduct any premiums due and not yet received from the claim payable by Viva Cover, if this policy is still in force. Viva Cover will not be obliged to accept premium tendered to it after the grace period.
Viva Cover shall at all times be entitled to increase the premiums payable in accordance with the terms of this Policy by way of a thirty (30) day written notice to the policyholder prior to the implementation of the increase. The premiums will increase annually by 8% on the anniversary of the policy and my premiums are guaranteed for a period of twelve (12) months.
I am entitled to terminate this policy within fourteen (14) days after receipt of my policy document by submitting a request to Viva Cover at firstname.lastname@example.org or by calling 0860 109 529. Viva Cover shall refund all premiums received prior to the receipt of the termination request into my designated bank account, subject to deduction of any costs of cover enjoyed during this period. This period is referred to as the Cooling Off Period.
